Rhinoplasty is one of the most popular surgeries for Asian people especially Thais because it can build confidence and good personality. There are 2 Rhinoplasty surgical methods as follows;

Synthetic material is used for this method such as silicone which will be inserted into open little wound at nose. This technique takes just a short time. Also, it is easy and comfortable. However, it is suitable for patients who have good nose shape For example, nose base is not too big, nose bridge is not too flat, and nose shape is not twisted and too long or too short because if its length is suitable for rhinoplasty, silicone can be inserted but it’s not suitable for the short one.

Advantages:

• It is not too complex surgical technique and takes short time.
• No general anesthetic needed but local one
• Low cost

Disadvantages:

• It is risky for the short nose because it could pierce through in the future.
• Some of nose structure cannot be reconstructed.

It is open Rhinoplasty method that surgeon will cut Culumella, which is located under the nose. Moreover, it is not only to reconstruct the nose bridge, but it can also reshape cartilage at the nose tip to be smaller and it will be done together with Septoplasty. Material to be used for this method is cartilage from patients’ body such as auricular or rib cartilage. Also,Ala Nasi can be reshaped and nose base can be pointy reconstructed. If patients have too wide nose base, it can be reconstructed by inserting Gore-Tex or silicone at the nose bridge which is safer from piercing through in the future.

Nose reconstructing with Gore-Tex is different from silicone. It will be more harmoniously attached with tissue than silicone and it is also not resisted by body.

Advantages:

• Nose tip can be reconstructed without piercing risky.
• Nose bone structure can be beautifully shaped.

Disadvantages:

• Takes longer time by specialist.
• General anesthetic is needed for surgical technique.
• Patients with some chronic diseases are not recommended to do with this technique.

การปรับโครงสร้างจมูก Reconstruction คือวิธีการผ่าตัดแบบเปิดแผลด้านล่างจมูก (Open Rhinoplasty) ซึ่งแพทย์จะกรีดแผลบริเวณแกนเนื้อใต้จมูก (Culumella) วิธีการนี้นอกจากจะใช้ในการเสริมสันจมูกแล้ว ยังสามารถตัดตกแต่งกระดูกอ่อนปลายจมูกโดยทำให้มีขนาดเล็กลง และเสริมปลายจมูกด้วยกระดูอ่อนร่วมกับการต่อขากลางหรือผนังกั้นกลางจมูก ให้มีความยาวเพิ่มมากขึ้น ซึ่งวัสดุที่ใช้เสริมจะนำมาจากร่างกายของคนไข้เอง เช่น กระดูกอ่อนหลังหู กระดูกอ่อนซี่โครง (ขึ้นอยู่กับแพทย์ประเมิน โดยส่วนใหญ่ใช้เพียงกระดูกหลังใบหูก็เพียงพอ) รวมไปถึงการผ่าตัดปรับรูปร่างปีกจมูกให้สวยงามและการเหลากระดูกฐานจมูกให้เรียวเล็กมากขึ้นหากคนไข้มีปัญหาฐานจมูกกว้าง ในบางเคสอาจไม่ต้องเสริมเพิ่มด้วยวัสดุสังเคราะห์ใดๆเลยก็ได้ ขึ้นอยู่กับความเหมาะสมจากการประเมินของแพทย์ แต่หากความสูงของสันจมูกไม่เพียงพอ แพทย์จึงจะเลือกใช้ซิลิโคนหรือ Gore-Tex มาเสริมเฉพาะด้านบนที่เป็นสันจมูกเท่านั้น จึงไม่เสี่ยงต่อการที่จมูกทะลุในอนาคตเนื่องจากบริเวณปลายจมูกของคนไข้จะเป็นกระดูกอ่อนทั้งหมด ซึ่งเรียกได้ว่าเป็นเทคนิคที่ได้รับความนิยมในประเทศเกาหลีอย่างมาก
